Inappropriate Antidiuresis: Examples of an Hyponatremic Syndrome Resembling Exogenous Yasopressin Administration in Man

چکیده
Hyponatremia has been recognized as a clinical entity since the finding by Loeb in 1932 that significantly lowered serum sodium and chloride levels are a characteristic finding in Addison's disease. It soon became apparent with the work of Winkler and Crankshaw in 1938, that hyponatremia was an accompaniment of diverse disease states as repeatedly confirmed since the introduction and clinical application of flame photometry for sodium analyses (Berry, Chappell, and Barnes, 1946; Fox and Baer, 1947; Hald, 1947; Bowman and Berliner, 1949; Wallace et al., 1951). Early papers emphasized the occurrence of hyponatremia in the two broad categories of cerebral and pulmonary disease (Peters et al., 1950; Sims et al., 1950; Welt et al., 1952; Cort, 1954); additional reports indicated a frequency of occurrence in tuberculous meningitis (Rapoport, West,

Nephrogenic syndrome of inappropriate antidiuresis.
The nephrogenic syndrome of inappropriate antidiuresis (NSIAD) is a rare, recently recognized disorder in water balance affecting not only infants but also adults. A new molecular mechanism responsible for NSIAD has recently been identified: a gain of function of the arginine vasopressin (AVP) receptor type 2 (V2R), causing the constitutive activation of the receptor. Clinical presentation and ...
متن کاملInterferon-alpha is a predisposing risk factor for carbamazepine-induced hyponatremia: A case of syndrome of inappropriate antidiuresis caused by interferon-alpha therapy
A 31-year-old man had been treated with carbamazepine (CBZ) for 6 years and warfarin with bucolome for 2 years before developing hyponatremia 7 days after an injection of interferon-alpha 2b and starting oral ribavirin for chronic hepatitis C virus infection. Despite the hyponatremia, urinary osmolality exceeded plasma osmolality, and urinary excretion volume decreased markedly after water load...
متن کاملHyponatremia revisited: translating physiology to practice.
The complexity of hyponatremia as a clinical problem is likely caused by the opposite scenarios that accompany this electrolyte disorder regarding pathophysiology (depletional versus dilutional hyponatremia, high versus low vasopressin levels) and therapy (rapid correction to treat cerebral edema versus slow correction to prevent osmotic demyelination, fluid restriction versus fluid resuscitati...
متن کاملNephrogenic syndrome of inappropriate antidiuresis in adults: high phenotypic variability in men and women from a large pedigree.
Nephrogenic syndrome of inappropriate antidiuresis (NSIAD) is a recently described genetic cause of hyponatremia in male infants. Whether this X-linked condition could be detected in the adult or also could affect women is unknown. A large five-generation family was identified in which the recently described arginine-vasopressin receptor type 2 (AVPR2) mutation that is responsible for NSIAD was...
متن کاملLong-Term Outcomes in a Family with Nephrogenic Syndrome of Inappropriate Antidiuresis
We report a familial case of the nephrogenic syndrome of inappropriate antidiuresis (NSIAD), including 30-year followup data on two patients. The proband and one maternal uncle presented in their infancy with severe recurrent hyponatremia, and clinical pictures consistent with the syndrome of inappropriate antidiuretic hormone (SIADH) in the absence of an elevated ADH level. They were both conf...
